"Why mull on death (of form)" Because it opens the window of our mind to think who we are? From where we have come ? why we are here and where shall we go ? An animal lives only in present and does not think about such questions. Answer to these questions elevates our lives and makes it meaningful.

Judging by the high number of military personnel who after returning from those places of destruction need psychiatric treatment and even then go on to commit suicide seems proof of what they realise they've done has nothing to do with a higher cause.

fear concerning it reduces. All the more when we have done pious work, not harmed anyone and lived a duty bound spiritual life we will have no fear of death and could face it confidently.

From the moment we are born, we know that we will die someday.
Fear of how we meet the death really is not that important.

Fear comes from knowing that we would leave this physical world too early when we are not ready to say good bye to all of our accumulated physical and emotional attachments.
Hence the reason, those w/o accumulated attachments (such as monks, priests, nuns, those w/o any family nor pets, and etc.) do not have any fear of death.

About your categories of different life choices meeting different degree of peaceful/violent death, I disagree with you 100%.

Here are the reasons:
Peaceful death at home while you sleeping is due to mostly cardiac failure or stroke. This can happen to most notorious criminals and those who do not believe anything spiritual.

Also, someone commits suicide by overdosing, it is not painful nor violent death. This certainly does not fit into the loving life and God category at all.

About most violent death, historically, many of monks, missionaries, nuns, peaceful social activists, and etc who just want to help humanity were violently executed (some were even tortured before the mercy of death).

So, one can be as humble, spiritual, God fearing, self sacrificing, pure goodness in heart, and etc. ;however, violent and painful death still happens to them as well.
Especially those who do not take care of their body and mind will suffers at hospital before their death - even those with most holy life choices.

Just take care of our bodies and mind as best as we can so our bodies do not suffer from preventive diseases before our death.
Eat healthy, don't use substances, don't become alcoholic, don't smoke, exercise modesty, think positively, get your physical checkups, use common sense with your adventures, etc. etc.

The way we die is the same as the way we are born. - it is written and destined.

Soldiers die fearlessly is not true at all.
Soldiers are BRAIN WASHED before sent to kill others in name of whatever. - either that or they are forced.
They fear just as same as others.

Monks die fearlessly because they have absolutely zero attachments to this physical world. Most monks are not really living and are not experiencing their physical reincarnations. They just exist until they die.
Unlike nuns and missionaries in other religions, most monks do not even contribute anything to humanity. It is all about them trying to go back to the spiritual land where we all came from and where we all go back to anyway.
Waste of physical incarnation is what monk life is.
I say here, most monks but not all monks.
Some monks are spiritual leaders and try to bring enlightenment to humanity. - Like Dalai

You are trying to say that the death is some accident and it has little to do with our Karma.

I have cited the example of Christ as No. 1 type death despite agonizing. When some one has all First Class Karma like Lord Rama he deserves to write his death script. Most Yogies leave their bodies fearlessly as per mode, time and place of their choice.This kind of power they acquire because of their good Karma. Though some criminals when shot down, meet instant death perhaps painless, or by cardiac failure the credit should definitely go to their past good Karma.

Personally, I would like to disappear without leaving any physical remains for which I depend on my good Karma and prayer.
