Have you ever managed to do it while awake?

Not yet, but who knows ...

I know the sleep paralysis very well. I have experienced this paralysis state (i can't give you the exact date, because i haven't taken notes on it. but it feels like this body was at 10~12 years).
The first experiences with the paralysis are always scary, you suddenly wake up and can't move. Fear makes you see things which are not there. (of course there are things which you see with the spiritual vision, but fear makes them fearful.)
Back then i have learned how to get out of the state. I know exactly what i need to do, if i want to get out of the paralysis. And somehow the interest in AP'ing ceased.
But later, the interest came back, and the paralysis state came back. The fear was there, and first few times i just wanted to wake up.
But then, the curiosity made me think and reason against the fear.
The reasoning was something like this: I was alone, in the room, there can't be anything else in it, why am i experiencing this fear ?
I told to myself that i was safe and secure. Eventually the fear subsided until it was gone, and i was able to lay there, in the paralysis, observing the effects.
(because waking up in the sleep paralysis does not happen that often, i was talking about experiences which took a span of about 1~2 years)

I can't help you with the fear, because it is a part of your thinking which causes it. And only you are the master of your thoughts.
I can share only few experiences.
Try to find out what is causing the fear. Where does it come from, and why.
When you get into the fearful state, look (with your thoughts) where is this fear generated/originated.
This searching for the source of fear usually eliminates it. (because it is not real, but this you have to experience for your self)
When you do this, soon or later the fear will be gone. But you have to look and to reason against it. (do not use aggressive reasoning, but sane)
Hint: You said that you have survived something ... When you get into the state, look if the fear of the repeated pain is the cause of the fear to go further.
Hint #2: Fear comes from thoughts. From misdirected thoughts. If you do not want to do the last step, your thoughts may ...
