What I can say is "YES" to all your "would" questions. I've experienced them all due to not checking their work & giving more trust than they deserved as well as not trusting my own instincts. I've gotten into serious issues from not knowing the correct words to use to disagree or fire someone, instead I worried too much about not hurting their feelings.

As for myself, I loathe conflict & tried to avoid it as much as possible. I've found one way to diffuse the situation by asking the other person enough questions to clarify their accusation in turn it reminds me of important details to properly defend myself - it works. It's only about improving skills, not about finding fault in yourself. Seriously, there are times when it's just the other person's/employer's temperament, lack of communication skills or just wanting what s/he wants to make the problem go away. All those can be intentional when it's known the opponent can buckle easily under pressure.

Store cashiers/clerks are much different in that their actions are nowhere as likely, if at all, to wreck havoc or cause harm or injury. All one has to is watch the cash register screen as items are scanned or check your receipt every time. Sometimes sale prices are not immediately entered into their system or for some reason do not show up even though they are tagged w/a sale price on the shelf.

No matter what you do throughout life situations will arise that test you to improve yourself.
