Can tarot tell someone's true thoughts and feelings towards you?
Can tarot tell someone's true feelings, how they feel deep down towards someone? Not just thoughts on the surface?
If it can, what spread should I use?

Like any medium, tarot relies on the skill and intuition of the reader. But yes, it has the potential to get into the nitty gritty.

It's subjective.
It depends how invested you are in the outcome.

Divination at it's best is a read of current energies, given that people are in a constant change of evolution or devolution, tarot, or any divination form, looses accuracy as energies change and evolve.
Meaning a reading will be of what that person currently feels deep down....which is subject to change.

If tarot can only read how the person currently feels, can I do it once a week? Since someone's feeling can be different from last week to this week.

It absolutely can, and with experience you will be able to tell a lot more than just feelings with just a few cards. You will not have to draw complicated spreads for that. It depends, if you are just starting you can do anything between drawing just one card and interpreting it for a longer time, to drawing a three card spread or some of the popular spreads and interpreting them as well. It takes practice, do what feels right to you. As long as you are willing to invest time to read them, it should be good.
If you only draw one card for the other person and try to read just that one card, it can tell you a lot itself. Then you can draw more cards depending on what more you want to know.

Do I have to follow the traditional meanings of the cards?
For example, people say The Fool is confident, but I feel the fool is scared and avoiding something when I just look at the card?

No, I never read any traditional card meanings. Like never. If you feel it is scared and avoiding something. Then that's it.
