from my own experience as a healer, in those patients who didn't make themselves into thinking into something specific, reiki sessions went more smoothly as their thoughts weren't interacting with the process of healing.
I have had a few cases where the patient tried very hard to help me heal him (in good faith) by concentrating on something but actually blocked the healing session by having incorrect view about how the healing process is ongoing.
my advice - tell the patient to just lie down and enjoy the present moment
