It sounds like the end result, the "symptom" is a cramped muscle under the shoulder blade. the fact that you had a headache the night before, then slept and awoke with this would suggest that you were defensive in your sleep, using your shoulder/neck to support your aching head while you slept, causing the cramping. Indeed water and rest are a good start. Energetically, there are many reasons it could have happened, and that's something you need to look within to find. Others have recommended going to a physician, and I do as well, just keep in mind there probably won't be much they can do for immediate relief, but they would be able to make sure there is no damage.
